 As a way to maximize time, please fill out the Consent for Treatment Form below prior to your first session. 

Please print and bring completed form with you to your first appointment as this needs to be included in your file. 

Please keep in mind, that in case of a minor, both parents need to sign it.

Forms will also be available to be completed in-office. 

Dr. Quintas will review and answer any questions you may have during your first visit. Any question you may have, is a good question!
